About The Position

Paradigm Health is seeking a full-time Hospice RN for an on-call schedule from Friday through Monday in Indiana. The role responds to on-call needs within an hour and provides physical and psychosocial assessment and care for hospice patients, supporting patients and their families through the physical, mental, social, and emotional needs associated with terminal illness. The nurse assesses and reassesses pain, uses appropriate pain-management techniques, and educates patients and families about pain management. The RN assesses patient condition, initiates and updates plans of care, obtains physician orders, and collaborates with physicians and the multidisciplinary team. Requirements

At least one year of RN experience.
Licensed Registered Nurse in the State of Indiana.
Current driver's license.
Reliable transportation and insurance.

Nice To Haves

One year of hospice experience is preferred.

Responsibilities

Respond to on-call needs within an hour.
Perform physical and psychosocial assessments of hospice patients.
Assess and reassess pain, use appropriate pain-management techniques, and educate patients and families about pain management.
Respond to the physical, mental, social, and emotional needs of terminally ill patients and their families.
Assess patient condition, initiate a plan of care, and re-evaluate and update it as necessary.
Obtain necessary physician orders and collaborate with physicians as patient needs dictate.
Initiate case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, and related matters.
