About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The service area includes Damariscotta and coastal areas. The position includes patient and family education, accurate and timely documentation, symptom-management monitoring, communication with the clinical manager and care team, and assigned on-call responsibilities. The role offers a $10,000 sign-on bonus. The annual salary range is $75,620 to $113,430 based on full-time employment. Requirements

Current RN license specific to the state(s) where assigned to work.
At least 1 year of nursing experience.
Current CPR certification.
Valid driver’s license, reliable transportation, and liability insurance.
Pass a drug test before beginning employment.

Responsibilities

Assess patients and collaborate with the care team to develop and implement a plan of care.
Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and in accordance with care center policies.
Record changes and outcomes related to pain and symptom management as appropriate.
Teach patients, families, and caregivers appropriate patient care techniques.
Communicate regularly with the clinical manager and care team.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Perform other duties as assigned.
