About The Position

Beacon Hospice, part of Amedisys/Optum, is hiring a Hospice Registered Nurse Case Manager to provide hospice care to patients at home. The role offers a stated salary range of $92,000 to $100,000. The nurse assesses patients and works with the care team to develop and implement individualized plans of care, provide education to patients and caregivers, and maintain accurate patient records. The position also includes on-call responsibilities as assigned.

Requirements

• One year of nursing experience.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately and on time, according to care center policies.
• Record changes and outcomes related to pain and symptom management, as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
