Hospice Nurse RN Case Manager
About The Position
The Hospice Registered Nurse provides individualized nursing care to maximize comfort and quality of life for hospice patients. Working at the center of an interdisciplinary care team, the RN builds relationships with patients, families, physicians, and teammates while practicing with autonomy and organizational support. The role includes assessing patients’ physical, emotional, social, and spiritual needs; developing and carrying out care plans with the patient’s support network and interdisciplinary group; educating and supporting patients and caregivers; coordinating care; and documenting patient needs and progress. Participation in on-call or admission rotations may be needed, along with ongoing learning and professional development. The position is PRN and covers Beaumont and surrounding areas. Pay is $40.00–$43.00 per hour.
Requirements
- Registered Nurse (RN) license in the state of practice.
- At least one year of nursing experience.
- Reliable transportation, a valid driver’s license, and appropriate auto insurance.
Responsibilities

- Perform comprehensive physical, emotional, social, and spiritual assessments to guide care decisions.
- Educate and support patients and caregivers.

- Document care clearly and accurately to reflect patient needs and progress.
- Participate in on-call or admission rotations as needed.
