RN Hospice Case Manager Nurse
About The Position
Compassionate Care Hospice, an Amedisys Company, is hiring an RN Case Manager based in The Villages, Florida. The position requires daily travel around Sumter County and follows a Monday–Friday schedule from 8:00 AM to 5:00 PM. The nurse provides hospice care through patient assessments, care planning, symptom management, education, documentation, and coordination with patients, families, and the care team. New hires continue onboarding alongside nurses in the program. RN orientation averages 7–9 weeks and is individualized according to the nurse’s needs and experience. The position requires a background screening through the Florida Care Provider Background Screening Clearinghouse.
Requirements
- Current RN license in the state or states where the employee will work.
- At least 1 year of clinical RN experience in an acute healthcare setting.
- Current CPR certification.

- Comfort with daily travel between facilities.
- Proficiency with electronic medical record (EMR) systems.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain accurate and timely individualized patient care progress records in accordance with care center policies.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
