About The Position
The RN Program Coordinator at ProMedica coordinates collaborative patient care from hospital discharge through follow-up. The role facilitates the patient plan of care, provides education and instructions for procedures and testing ordered by providers, and reviews patients’ Health Risk Assessments. The coordinator monitors patient charts for test results and reviews results with providers and patients, and reviews and manages preoperative surgical clearance requests with providers when needed. The role may also involve triaging patient calls and providing health advice and referrals as appropriate. Requirements
- Current state license as a Registered Nurse.
- One year of relatable or equivalent experience.
- ACLS certification.
- BLS certification.
Nice To Haves
- Bachelor of Science in Nursing.
- EPIC experience.
- Two or more years of relatable experience.
