About The Position
The RN Program Coordinator at ProMedica is the point person for coordinating collaborative patient care from hospital discharge through follow-up. The role facilitates the patient plan of care, provides education and instructions for provider-ordered procedures and testing, and reviews the patient’s Health Risk Assessment. The coordinator monitors patient charts for test results and reviews results with providers and patients. The role also reviews and manages preoperative surgical clearance requests with providers when needed, and may triage patient calls, provide health advice, and make referrals as appropriate. Requirements
- Current state license as a Registered Nurse.
- One year of relatable or equivalent experience.
- ACLS certification.
- BLS certification.
Nice To Haves
- Bachelor of Science in Nursing.
- EPIC experience.
- Two or more years of relatable experience.
