RN Case Manager - $1500 Sign on Bonus

About The Position

The RN Case Manager assesses the impact of terminal disease on patients’ physical, psychosocial, and environmental needs and activities of daily living. The role implements and recommends revisions to individualized plans of care, coordinates care with patients, caregivers, physicians, and other healthcare professionals, and participates in interdisciplinary care planning. The position also includes assigned on-call responsibilities, timely documentation, and compliance with applicable laws, Medicare regulations, and organizational policies. A $1,500 sign-on bonus is offered.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current State Registered Nurse License.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.
Two years of RN experience in a healthcare or clinical setting preferred.

Responsibilities

Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal considerations.
Implement the individualized plan of care and recommend revisions as necessary.
Collaborate on care with patients and caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group meetings, coordinate and participate in patient care planning and prioritization of needs, and collaborate with the group to promote coordinated care.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
