About The Position

As a Floating RN Case Manager, you'll rotate throughout different portions of our 21-county service area, stepping into territories where additional clinical coverage is needed. Your assignments will typically last approximately three months, giving you the opportunity to become part of each territory you support. This role offers the chance to learn patients, teams, and areas, manage caseloads, and apply that learning in new environments, resulting in a home health nurse with a broad range of experience. Every territory teaches you something new, exposing you to different patient populations, clinical challenges, and perspectives. This role strengthens your ability to quickly understand situations and take ownership, providing exposure that can be difficult to develop in a single territory. The rotations are based on patient census, staffing, and clinical coverage needs, offering variety with enough consistency to be effective.

Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Strong comprehensive assessment and clinical decision-making skills
  • Confidence independently managing a patient caseload
  • Strong organization and time-management abilities
  • Excellent patient, caregiver, physician, and team communication
  • Ability to adapt quickly to new teams and territories
  • A collaborative, flexible approach to patient care
  • Reliable transportation
  • Willingness to travel throughout our 21-county service area as territory assignments change

Responsibilities

  • Manage an assigned home health caseload within your current territory
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Perform comprehensive nursing assessments
  • Develop and update individualized plans of care
  • Provide skilled nursing visits
  • Perform wound care, medication management, and other skilled nursing services
  • Educate patients and caregivers
  • Monitor progress throughout the episode of care
  • Identify changes in condition and intervene appropriately
  • Coordinate care with physicians and other healthcare providers
  • Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
  • Help reduce avoidable hospitalizations and support patients in remaining safely at home
  • Maintain timely and accurate documentation in Homecare Homebase (HCHB)
