About The Position
The Registered Nurse (RN) Case Manager is a full-time, out-patient position serving the Houston area. The role requires an independent and experienced RN and is an integral part of the interdisciplinary care team. The RN Case Manager is responsible for admitting patients and coordinating appropriate care for each patient and family. Work hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. On-call duties and weekend rotation may be required.
Requirements
- Active RN license in the State of Texas.
- Effective written and verbal communication skills.
- Ability to travel and reliable transportation.
- Clean background, drug screen, and driving record.
Nice To Haves
- At least 1 year of experience as an RN.
- Experience with hospice or home health.
Responsibilities
- Admit patients.
- Coordinate appropriate care for each patient and family as part of the interdisciplinary care team.
- Conduct assessments.
- Manage discharges.
- Perform pronouncements.
