Hospice - Full Time - RN, Case Manager
About The Position
Dignity Hospice is seeking a full-time Registered Nurse Case Manager to provide hospice care in Richardson, Plano, Garland, Wylie, and Sachse, Texas. The RN case manager plans, organizes, and directs hospice care, drawing on community resources to meet the needs of patients and families in their homes and communities. The role serves as the central point for each patient’s care plan and helps lead and mentor Home Health Aides. The position works with physicians and other clinicians to support patient care plans and goals. The role is on the road and involves travel; 75% travel is preferred. The schedule is Monday through Friday, with weekend availability.
Requirements
- Current Texas State license as a Registered Nurse.
- At least 2 years of experience as a Registered Nurse in home health.
- Reliable transportation.
- Current Texas Driver’s License.
- Valid and current auto liability insurance.
- Kinnser EMR experience; 2 years required.
- Two years of hospice experience required.
- RN license required.
Nice To Haves
- One year of case management experience preferred.
- Two years of nursing experience preferred.
- Willingness to travel 75% preferred.
Responsibilities
- Perform comprehensive assessments for patient admission.
- Develop and update the plan of care in consultation with the physician and clinicians involved in the patient's care.
- Participate in the patient's discharge planning process.
- Provide skilled nursing care as needed in accordance with the plan of care.
- Coordinate with other agencies providing nursing or related services to ensure continuity of care and implement a comprehensive care plan.
- Pursue improvements in nursing care through in-service and formal education, workshops, conferences, and participation in professional and related organizations.
- Maintain the clinical record, including completing visit-note documentation within 24 hours, comprehensive assessments including OASIS within 48 hours, and timely updates to medication profiles and submission of orders and plans of care.
- Maintain confidentiality of patients’ private health information.
- Lead and mentor Home Health Aides.
