Hospice RN Case Manager - Full Time

About The Position

As an RN Case Manager at Luminos Hospice, you will work with the Clinical Director to lead a care team that includes LPNs, LVNs, aides, an MSW, and a chaplain. You will coordinate and oversee patients’ home care, collaborate with the care team to develop and implement care plans, and provide education and support to families. The role includes clinical assessments, medication and catheter management, timely documentation, supervision of HHAs, and regular communication about patient progress.

Requirements

Current state license as a Registered Nurse.
Current driver’s license.
Current CPR certification.
Reliable transportation and valid auto liability insurance.
Nursing skills consistent with generally accepted standards of practice.
Knowledge of state and federal regulations.
Ability to work independently and make sound decisions.
Strong attention to detail and organizational habits.
Excellent verbal and written communication skills.
Good interpersonal skills.

Nice To Haves

Two years of experience as a Registered Nurse in a clinical care setting.
One year of full-time hospice experience.

Responsibilities

Coordinate and oversee all aspects of patients’ home care.
Collaborate with patient care teams to develop and implement care plans.
Educate families about patients’ conditions, needs, and care.
Perform head-to-toe physical assessments.
Check vital signs and manage medications.
Administer and maintain catheters.
Complete documentation accurately and on time.
Supervise HHAs.
Regularly communicate patient progress to the clinical manager, physician, and care team.
Perform other duties as assigned.
