About The Position

The Respiratory Therapy Discharge Planner ensures safe, effective, and timely transitions of care for patients with respiratory conditions. The role coordinates discharge planning activities, collaborates with interdisciplinary teams, and facilitates continuity of care through patient assessment, direct clinical support, equipment coordination, education, and follow-up services. It provides clinical expertise in developing and implementing respiratory care plans and partners with care providers and respiratory leadership to promote quality, evidence-based practices that optimize patient outcomes.

Requirements

Associate or bachelor's degree in respiratory therapy.
Current licensure in good standing to practice as a Respiratory Care Practitioner in the state(s) of practice.
Registered Respiratory Therapist (RRT) credential from the National Board of Respiratory Care (NBRC).
Basic Life Support (BLS) certification.
Three years of experience as a staff therapist.
Clinic experience with adult patients.
Experience with discharge planning and coordination of respiratory durable medical equipment for the home.
Excellent communication and organizational skills.
