About The Position
The Discharge Planner assists patients, their families, or significant others who need help with social, economic, or environmental problems resulting from or related to illness. The role works closely with the interdisciplinary team to address complex social and discharge planning issues and collaborates with community agencies as indicated to meet patient needs across the continuum of care. The position has full access to patient health information.
Requirements
- Baccalaureate degree in social work, psychology, sociology, or another related field.
- At least one year of healthcare discharge planning or social work experience.
- Working knowledge of community social and health agencies.
- Knowledge of human behavior and personality dynamics.
- Ability to pass a basic FIT test involving bending, stooping, standing, and sitting for extended periods.
- Ability to lift and carry up to 30 pounds.
