About The Position

Promise Care is seeking a full-time Resident Care Coordinator for a 126-bed residential care facility. The coordinator helps provide quality resident care by ensuring residents’ safety and physical, emotional, spiritual, and medical needs are met. The role calls for leadership and customer service skills, and involves working with the Director of Care and the administrator to support staff and coordinate residents’ care. Responsibilities

• Work alongside the DOC and administrator.
• Monitor daily work assignments for completion and accuracy.
• Provide training and direction to staff in accordance with facility policy.
• Administer medications.
• Obtain residents’ weights and vital signs.
• Perform face checks and other protective oversight measures to ensure resident safety.
• Assist residents with personal care needs.
• Chart resident care.
• Interact with residents and address their needs and desires.
• Report changes in residents’ health and behavior.
• Coordinate residents’ care needs with providers and outside resources.
• Participate in on-call duties, staffing, and scheduling.
