About The Position
CLFC Healthcare and Communications is hiring a per diem Registered Nurse for an on-site role at Wayne County's 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility was rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025. The RN carries a resident assignment, directs LPNs and CNAs on the unit, and serves as charge nurse when assigned, with nursing leadership on site. This W-2 hourly role offers a self-selected schedule with no minimum hours. Nurses choose available shifts through the BRIDGE App and may work days, evenings, or nights; twelve-hour shifts may be available. Compensation is $52.00–$58.00 per hour, with nights and weekends paid at the top of the range. Overtime over 40 hours and all ten County holidays are paid at time and a half. Pay is weekly, and orientation is paid at the nurse's rate. CLFC reimburses New York license renewal and CPR renewal, supports continuing education and certifications, and offers up to $1,500 per year in tuition assistance toward a BSN. A $500 referral bonus is offered for bringing a nurse or aide onto the roster. Fingerprinting, immunizations, screening, and license verification are arranged and paid by CLFC. Paid sick leave accrues under New York law, and professional liability insurance covers each shift. A shift canceled by the facility two hours or less before its start is paid for four hours. The Program Manager provides a direct contact for scheduling and pay questions.
Requirements
- Current, unrestricted New York State Registered Professional Nurse license.
- Graduation from an accredited nursing program.
- Current BLS (CPR).
- One year of recent RN experience.
- Ability to complete the New York State Department of Health Criminal History Record Check and a drug screen.
- Current on immunizations required by New York for nursing home personnel.
Nice To Haves

Responsibilities
- Carry a resident assignment, including assessments, medication administration, treatments, and responding to changes in condition.
- Direct the LPNs and CNAs on the unit and serve as charge nurse for the unit when assigned.
- Maintain current MDS-related documentation, care plan updates, and shift reports in the facility's system.
- Act as the point of contact for residents, families, and providers during the shift.
- Follow the facility's infection control, safety, and sign-in practices.
