About The Position
As the Per Diem Registered Nurse, you will provide and direct nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies. You will coordinate each patient’s total plan of care with other health care professionals and support continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources. The role provides home health services through Almost Family, part of LHC Group and the Optum family of businesses. The position offers performance recognition and direction, as well as development opportunities for other roles. Candidates must pass a drug test before beginning employment.
Requirements
- Current and unrestricted RN licensure in the state of practice.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
Nice To Haves
- Current CPR certification or ability to obtain it within 90 days of employment.
- At least 1 year of RN experience.
- Ability to work independently.
- Good communication, writing, and organizational skills.
Responsibilities
- Provide high-quality clinical services within scope of practice and infection control standards.
- Coordinate care with other members of the patient’s care team from admission to discharge.
- Complete clinical nursing assessments according to federal and state program requirements and payer needs.
- Ensure patient eligibility and medical necessity for services as defined by payer and agency policy.
- Develop and revise individualized plans of care or service plans with other community providers.
- Ensure the frequency and duration of the plan of care meet patient needs and initiate revisions with physician approval.
- Function in home situations regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
