About The Position

Elara Caring is seeking a Registered Nurse to provide home health care. The role involves assessing patients’ care needs, delivering nursing services according to the plan of care and physician orders, and helping ensure safe, continuous, high-quality care. The nurse monitors patients’ conditions, reports changes to the care team, and educates patients and families about care and safety. The position requires travel between patient homes and assigned work locations. A $5,000 sign-on bonus is offered.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
