About The Position

The On Call Hospice Visit Registered Nurse provides compassionate, responsive care to hospice patients in Sussex and Morris Counties during the on-call hours of Monday through Friday, 5 p.m. to 8 a.m. The nurse addresses urgent patient needs, supports patients and families, and contributes to a comprehensive, responsive plan of care. The role works collaboratively with the RN Case Manager and Interdisciplinary Team (IDT), while operating under the supervision of a Registered Nurse. It combines independence with leadership and team support and requires accurate documentation and adherence to hospice care standards and protocols.

Requirements

Active and unrestricted RN license.

Responsibilities

Provide compassionate on-call care to hospice patients and address urgent needs with empathy and professionalism.
Collaborate with the RN Case Manager to implement tailored interventions that support patients and their families.
Maintain accurate and thorough documentation.
Contribute as a member of the Interdisciplinary Team (IDT) to a comprehensive and responsive plan of care.
Work under the supervision of a Registered Nurse to assess and meet the multifaceted needs of patients and their families.
Provide continuous, compassionate care in accordance with hospice care standards and protocols.
Help identify and address patients’ and families’ physical, psychological, social, and spiritual needs.
