About The Position

The On Call Hospice Visit Registered Nurse provides compassionate, responsive care to hospice patients and their families, addressing urgent needs with empathy and professionalism. The nurse collaborates with the RN Case Manager and Interdisciplinary Team (IDT) to implement tailored interventions and support a comprehensive, responsive plan of care. The role involves assessing and meeting patients’ physical, psychological, social, and spiritual needs, maintaining accurate documentation, and following hospice care standards and protocols. The position is described as full-time and covers Monday through Friday, 5 p.m. to 8 a.m., serving Ocean and Monmouth Counties.

Requirements

Active and unrestricted RN license.

Responsibilities

Provide continuous compassionate care in accordance with hospice care standards and protocols.
