About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Hospice RN On-Call provides after-hours clinical support, triages patient needs, and coordinates care to promote continuity and quality outcomes. Requirements

Current RN license specific to the state or states assigned.
Current CPR certification.
At least 1 year of nursing experience; hospice, home health, or acute care experience is preferred.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Provide on-call nursing support, triage patient needs, and determine appropriate interventions.
Assess patient conditions by phone and/or through in-person visits, and escalate care as needed.
Coordinate with physicians, care teams, and caregivers to support timely and effective care delivery.
Educate patients and caregivers about symptom management and care plans.
Maintain accurate and timely documentation of patient interactions and clinical decisions.
