About The Position

Concierge Home Care is hiring Home Health Registered Nurses to provide one-to-one skilled nursing care in patients’ homes in St. Augustine and St. Johns County, Florida. Territories are designed to minimize drive time through clustered scheduling. Nurses can choose full-time or PRN work and a weekday or weekend schedule, with flexible scheduling and autonomy. Compensation is pay per visit at $45–$51 per visit, plus Medicare differential pay; average full-time earnings are stated as $85,000 or more annually. The role also offers quarterly bonus opportunities, mileage reimbursement, and consistent patient volume.

The position includes patient assessment, individualized care planning, skilled nursing care and patient education, coordination with physicians and interdisciplinary teams, monitoring progress, and adjusting care plans. Documentation uses streamlined AI-assisted tools and the WellSky/Kinnser EMR. The company provides structured onboarding, mentorship, preceptorship, ongoing clinical training, free CEUs, and career advancement opportunities. Health insurance begins after 30 days, PTO accrues from day one, and a mileage and data stipend is provided according to company policy. Requirements

• Active, unencumbered Florida Registered Nurse (RN) license.
• Valid driver’s license, reliable transportation, and auto insurance.
