About The Position
Concierge Home Care is hiring Home Health Registered Nurses to provide one-to-one skilled care in patients’ homes in St. Augustine and St. Johns County, Florida. Territories are designed to minimize drive time through clustered scheduling. The role offers a pay-per-visit compensation model, flexible scheduling and autonomy, with consistent patient volume provided. Average full-time earnings are advertised as $85,000 or more annually. Nurses create and manage their own schedules and participate in a monthly on-call rotation for additional earnings. The position includes structured onboarding, mentorship, preceptorship with experienced clinical educators, ongoing training, free continuing education units, and career advancement opportunities. AI-assisted charting and the WellSky/Kinnser electronic medical record system are used to streamline documentation.
Requirements
- Active, unencumbered Florida Registered Nurse (RN) license.
- Valid driver’s license, reliable transportation, and auto insurance.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Perform patient assessments and develop individualized care plans.
- Provide skilled nursing care and patient education.
- Coordinate care with physicians and interdisciplinary teams.
- Monitor patient progress and adjust care plans as needed.
- Complete documentation using AI-assisted tools.
