About The Position
BAYADA Home Health Care is hiring a compassionate, dedicated Weekend Baylor Registered Nurse for full-time home health visits. The office serves adult and geriatric clients on a per-visit basis throughout Montgomery County and parts of Philadelphia. The weekend schedule is Friday through Sunday or Saturday through Monday. Home health experience is required in the opening description. The position involves providing care in clients’ homes, carrying out care plans, monitoring and documenting client conditions, and coordinating care. Requirements
- Current Registered Nurse license in Pennsylvania.

- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Ability to travel to assigned cases.

Nice To Haves
- Prior home care experience is strongly preferred in the qualifications section, although that section states it is not required.
- Prior experience with electronic medical records (EMR).
Responsibilities
- Follow designated care plans according to patients’ needs.
- Make home visits to clients in assigned geographic territories.
- Perform assigned duties, including medication administration, wound care, treatments, and procedures.
- Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
- Follow up on, execute, and properly document doctors’ orders.
- Perform client assessments as needed.
- Provide case management and coordinate care.
- Accurately document observations, interventions, and evaluations related to client care and services using a touch pad tablet.
