About The Position

BAYADA Home Health Care is seeking a compassionate and dedicated full-time Weekend Baylor Registered Nurse to provide per-visit home health care to adult and geriatric clients in Montgomery County and parts of Philadelphia. The weekend schedule is Friday through Sunday or Saturday through Monday. The role involves providing and coordinating patient care in clients’ homes, following care plans, documenting services, and traveling to assigned cases.

Requirements

• Current Registered Nurse license in Pennsylvania.
• At least one year of recent, verifiable clinical medical/surgical experience.
• Graduation from an accredited and approved nursing program, as indicated by a school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills.
• Ability to travel to assigned cases.

Nice To Haves

• Prior home care experience is strongly preferred.
• Prior experience with electronic medical records (EMR) is preferred.

Responsibilities

• Follow designated care plans in accordance with patients’ needs.
• Make home visits to clients in assigned geographic territories.
• Administer medication and provide wound care, treatments, and procedures as assigned.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors’ orders.
• Perform client assessments as necessary.
• Provide case management and care coordination.
• Accurately document observations, interventions, evaluations, and services using a touch pad tablet.
