About The Position

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse to provide per-visit care to adult and geriatric clients throughout Montgomery County. The position follows a Friday-through-Sunday weekend schedule and requires home health experience. The nurse makes home visits, provides and coordinates care according to each client’s needs, and documents services using a touch pad tablet. Paid training and ongoing clinical support are provided.

Requirements

• Current Registered Nurse license in Pennsylvania.
• At least one year of recent, verifiable clinical medical/surgical experience.
• Graduation from an accredited and approved nursing program, evidenced by a school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Ability to travel to assigned cases.

Nice To Haves

• Prior home care experience is strongly preferred but not required.
• Prior experience with electronic medical records (EMR) is preferred.
• Solid computer skills.

Responsibilities

• Assess clients, educate patients and/or their families, and monitor patients’ conditions.
• Follow designated care plans in accordance with patients’ needs.
• Make home visits to clients in assigned geographic territories.
• Perform assigned duties, including medication administration, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors’ orders.
• Provide case management and coordinate care.
• Accurately document observations, interventions, and evaluations related to client care and services using a touch pad tablet.
