About The Position

The Registered Nurse (RN) provides care and services to a group of patients by visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. The RN is a contributing member of the interdisciplinary team, providing expertise in psychosocial assessments, interventions, and knowledge of community resources. The role may be in Case Management, Admissions, or a Resource position. The position is part-time weekend work on Saturday and Sunday, 8 a.m. to 5 p.m., and is described as a supplemental income opportunity.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state or states of practice.
• Ability to work collaboratively within a team environment.
• Ability to work independently and communicate by phone, email, and in writing.
• Understanding of hospice rules and regulations.
• Time-management skills to manage visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive across the service area, with reliable transportation, a valid driver’s license, and auto insurance.

Responsibilities

• Provide care and services to a group of patients.
• Visit patients in private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team by providing expertise in psychosocial assessments, interventions, and community resources.
• Work in a Case Management, Admissions, or Resource role.
• Coordinate visits, documentation, and meetings; respond to urgent needs by prioritizing and triaging multiple concerns.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
