About The Position

Interim HealthCare is seeking a Home Health Registered Nurse to conduct weekend OASIS visits in Topeka, Kansas, and surrounding areas. The role focuses on patient-centered care, including OASIS assessments, skilled nursing visits, individualized care planning, coordination with other healthcare professionals, regulatory compliance, and clinical documentation. Nurses work with autonomy and 1:1 nurse-to-patient ratios, with support from management and an interdisciplinary team.

Requirements

• Graduate of an accredited nursing program.
• Active RN license in Kansas.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing specialty.
• Strong communication skills and compassion.

Responsibilities

• Complete accurate and timely OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments.
• Develop, implement, and update individualized patient care plans.
• Perform skilled nursing visits, including assessments, medication management, wound care, and patient education.
• Coordinate care with physicians, therapists, and interdisciplinary team members.
• Ensure compliance with Medicare, state, and agency regulations.
• Maintain thorough and timely clinical documentation in the EMR.
• Provide education and support to patients and caregivers.
