About The Position

Requirements

Responsibilities

• Provide direct patient care and nursing assessments for residents in an LTC/assisted living setting.
• Administer medications and monitor responses, document in resident charts.
• Perform routine nursing procedures, wound care, and assistance with activities of daily living.
• Develop and update individualized care plans in collaboration with the interdisciplinary team.
• Monitor resident safety, respond to changes in condition, and communicate with physicians and families as needed.
• Ensure compliance with facility policies, infection control practices, and documentation standards.
