About The Position

BAYADA Home Health Care is hiring a Registered Nurse to provide senior living visits for its Londonderry office, serving Dover, Exeter, and Rye, New Hampshire. The role offers flexible schedules, one-to-one care, and an emphasis on work/life balance. Nurses provide care according to designated plans, visit clients in assigned geographic territories, perform clinical treatments, monitor and document client conditions and care, and coordinate case management. The position requires a current New Hampshire RN license and at least one year of recent, verifiable medical/surgical clinical experience. Prior home care experience is preferred but not required.

Requirements

• Current license as a Registered Nurse in New Hampshire.
• At least one year of recent, verifiable clinical (medical/surgical) experience.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills.
• Ability to travel to cases as assigned.

Nice To Haves

• Prior home care experience is strongly preferred but not required.
• Prior experience with electronic medical records (EMR) is preferred.

Responsibilities

• Follow a designated care plan in accordance with the patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including medication administration, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors' orders.
• Perform client assessments as necessary.
• Provide case management and coordination.
• Accurately document observations, interventions, and evaluations related to client care management and services provided using a touch pad tablet.
