About The Position

BAYADA Home Health Care is hiring a Registered Nurse for senior living visits through its Londonderry office, serving Salem, Londonderry, and Nashua, New Hampshire. The position provides one-to-one care with flexible schedules and an emphasis on work/life balance. Nurses follow designated care plans, make visits to clients, provide nursing treatments, monitor and document client care, and coordinate case management. Requirements

• Current Registered Nurse license in New Hampshire.
• At least one year of recent, verifiable clinical medical/surgical experience.
• Graduation from an accredited and approved nursing program, evidenced by a school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Ability to travel to assigned cases.

Nice To Haves

• Prior home care experience is strongly preferred but not required.
• Prior experience with electronic medical records (EMR) is preferred.
• Solid computer skills.

Responsibilities

• Follow designated care plans in accordance with patients’ needs.
• Make home visits to clients in assigned geographic territories.
• Perform assigned duties, including medication administration, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors’ orders.
• Perform client assessments as necessary.
• Provide case management and coordination.
• Accurately document observations, interventions, and evaluations related to client care and services using a touch pad tablet.
