About The Position

The Registered Nurse provides home health care to patients in Miami County, Ohio, and surrounding areas. The RN manages patient care from admission through discharge, performs OASIS assessments, develops individualized plans of care, and collaborates with an interdisciplinary team to support positive patient outcomes. The role offers autonomy and flexibility while helping patients recover safely and comfortably at home. The position is listed as full-time, part-time, or PRN. The employer offers weekly pay, pay per visit, paid time off, medical, vision, and dental benefits, a sign-on bonus, weekly gas incentives of up to $500 per month, continuing education, supportive clinical leadership, manageable caseloads, and technology and documentation support. The role does not have an on-call rotation.

Requirements

Current Ohio RN license.
Current CPR and/or BLS certification.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong assessment, communication, organizational, and time management skills.

Nice To Haves

Experience in home health and OASIS assessments is preferred but not required.
Knowledge of Medicare home health regulations and documentation requirements is preferred but not required.

Responsibilities

Perform Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
Conduct comprehensive nursing assessments and develop individualized plans of care.
Coordinate and manage patient care throughout the episode of care.
Provide skilled nursing services, patient education, medication management, and disease management.
Collaborate with physicians, therapists, social workers, and other members of the interdisciplinary team.
Monitor patient progress and adjust care plans as needed.
Complete accurate and timely OASIS documentation and clinical records in accordance with Medicare, state, and agency guidelines.
Participate in quality improvement initiatives and maintain compliance with regulatory standards.
