About The Position

The Registered Nurse (RN) provides home health care to patients in Mercer County, Ohio, and surrounding areas. The role manages patient care from admission through discharge, completing OASIS assessments, developing individualized care plans, and collaborating with an interdisciplinary team. It offers autonomy and flexibility, with the opportunity to build meaningful relationships with patients and help them recover safely and comfortably at home. Requirements

Current Ohio RN license.
Current CPR and/or BLS certification.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong assessment, communication, organizational, and time management skills.

Nice To Haves

Home health experience and experience with OASIS assessments are preferred but not required.
Knowledge of Medicare home health regulations and documentation requirements is preferred but not required.

Responsibilities

Perform Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
Conduct comprehensive nursing assessments and develop individualized plans of care.
Coordinate and manage patient care throughout the episode of care.
Provide skilled nursing services, patient education, medication management, and disease management.
Collaborate with physicians, therapists, social workers, and other members of the interdisciplinary team.
Monitor patient progress and adjust care plans as needed.
Complete accurate and timely OASIS documentation and clinical records in accordance with Medicare, state, and agency guidelines.
Participate in quality improvement initiatives and maintain compliance with regulatory standards.
