About The Position

The Registered Nurse provides home health care to patients in Clark County, Ohio, and surrounding areas. The RN manages patient care from admission through discharge, completing OASIS assessments, developing individualized care plans, and coordinating with an interdisciplinary team. The role offers autonomy and flexibility while supporting patients’ recovery and safety in their homes. The position includes skilled nursing services, patient education, medication and disease management, progress monitoring, and accurate clinical documentation in accordance with Medicare, state, and agency guidelines. Home health and OASIS experience are preferred but not required.

Requirements

Current Ohio RN license.
Current CPR and/or BLS certification.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong assessment, communication, organizational, and time management skills.

Nice To Haves

Experience in home health and OASIS assessments is preferred but not required.
Knowledge of Medicare home health regulations and documentation requirements is preferred but not required.

Responsibilities

Perform Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
Conduct comprehensive nursing assessments and develop individualized plans of care.
Coordinate and manage patient care throughout the episode of care.
Provide skilled nursing services, patient education, medication management, and disease management.
Collaborate with physicians, therapists, social workers, and other interdisciplinary team members.
Monitor patient progress and adjust care plans as needed.
Complete accurate and timely OASIS documentation and clinical records in accordance with Medicare, state, and agency guidelines.
Participate in quality improvement initiatives and maintain compliance with regulatory standards.
