About The Position

Centric Home Health and Hospice is seeking a dedicated, experienced Registered Nurse to provide hospice care during after-hours and weekends. The RN conducts patient visits, assesses hospice eligibility, and coordinates care across disciplines while supporting patients and families with end-of-life needs.

Availability is required from 5 p.m. Friday through 8 a.m. Monday. Care is provided in patients’ homes under varying conditions, with local travel and a flexible schedule. The role may involve exposure to infectious diseases, blood, and bodily fluids, as well as prolonged standing, walking, and lifting up to 50 pounds.

Requirements

• Graduate of an accredited nursing program; diploma, associate degree, or bachelor's degree accepted.
• Current Registered Nurse (RN) state license.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong communication and interpersonal skills.
• Two years of clinical experience.
• Ability to travel locally and work a flexible schedule.
• Ability to perform prolonged standing and walking and lift up to 50 pounds.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive patient assessments and ensure hospice eligibility criteria are met.
• Develop, implement, and revise individualized plans of care.
• Educate patients and families about disease processes, self-care, nutrition, and end-of-life needs.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Provide clinical direction to Hospice Aides and LPNs/LVNs.
• Maintain accurate and timely documentation of care and patient progress.
• Participate in the on-call rotation and respond promptly to patient needs.
• Follow infection control and safety procedures.
• Uphold agency policies and professional standards.
