About The Position
KeystoneCare is seeking a compassionate and dedicated Hospice On-Call Registered Nurse to provide weekend support for hospice patients and their families. The role ensures patients receive expert care and guidance during off-hours by helping manage symptoms, providing comfort, and supporting families through end-of-life care. As a member of the interdisciplinary hospice team, the nurse responds to urgent patient needs, collaborates with physicians and staff, and assists with care coordination to ensure seamless service delivery.
Requirements
- Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
- Excellent clinical assessment and symptom management skills.
- Strong communication and interpersonal skills.
- Ability to respond promptly and independently during on-call shifts.
- CPR certification.
- Driver's license and reliable vehicle.

- Experience in hospice or palliative care.
Responsibilities
- Respond promptly to on-call hospice patient needs during weekends.
- Assess patient conditions, manage symptoms, and provide necessary nursing interventions.
- Provide guidance and support to patients' families and caregivers over the phone or in person as needed.
- Collaborate with the hospice interdisciplinary team to ensure continuity of care.
- Document patient interactions, assessments, and interventions accurately and promptly.
- Adhere to hospice philosophy and policies in all aspects of care delivery.
- Admit patients who are hospice appropriate and interact with families to assess needs and treatment plans.
