About The Position

KeystoneCare is seeking a compassionate and dedicated Hospice On-Call Registered Nurse to provide weekend support to hospice patients and their families. The nurse responds to urgent needs during off-hours, manages symptoms, provides comfort, and supports families facing end-of-life care. As a member of the interdisciplinary hospice team, the nurse collaborates with physicians and staff and assists with care coordination to ensure continuity of service.

Requirements

• Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
• Excellent clinical assessment and symptom management skills.
• Strong communication and interpersonal skills.
• Ability to respond promptly and independently during on-call shifts.
• CPR certification.
• Driver’s license and reliable vehicle.

Nice To Haves

• Experience in hospice or palliative care.

Responsibilities

• Respond promptly to on-call hospice patient needs during weekends.
• Assess patient conditions, manage symptoms, and provide necessary nursing interventions.
• Guide and support patients’ families and caregivers by phone or in person as needed.
• Collaborate with the hospice interdisciplinary team to ensure continuity of care.
• Document patient interactions, assessments, and interventions accurately and promptly.
• Adhere to hospice philosophy and policies in all aspects of care delivery.
• Admit patients who are hospice appropriate and work with families to assess needs and treatment plans.
