About The Position
Provide skilled nursing care to patients in their homes through intermittent visits, private duty shifts, and care plan coordination. The role includes skilled nursing visits and shift-based care for patients who need extended clinical support. Clinicians document patient condition and progress, collaborate with physicians and interdisciplinary teams, and follow agency protocols and applicable documentation and infection-control standards. Requirements
- Active Florida RN or LPN license; Compact License accepted.
- Current CPR certification (BLS).
- Level 2 background screening compliant with AHCA requirements.
- TB screening and physical completed within the past 6 months.
- Familiarity with EMRs, such as Axxess.
- Knowledge of PDGM and OASIS for RNs.
- Excellent clinical judgment, communication skills, and time management.
- Reliable transportation and commitment to punctual, professional care.
Responsibilities
- Perform skilled nursing visits, including wound care, IV administration, medication management, and patient assessments.
- Provide shift-based care for patients needing extended clinical support.
- Monitor and document patient condition and progress.
- Collaborate with physicians and interdisciplinary teams to update care plans.
- Maintain accurate records in compliance with HIPAA and CMS guidelines; RNs must complete OASIS documentation.
