About The Position

Elite Care Management is seeking a Registered Nurse or Licensed Practical Nurse to provide one-on-one home health care to a patient in Riverwoods. The nurse will work with one patient for the complete shift, get to know the patient and family, learn about the patient’s needs at home, and plan, organize, and assist with home health care services. The role includes assisting with daily living activities, ensuring patient comfort, and supporting overall well-being and quality of life. This is a part-time position with Tuesday and Thursday shifts from 7 a.m. to 7 p.m. Paid orientation and patient training, weekly pay by direct deposit, 24/7 on-call support, and access to a hands-on training lab are offered.

Requirements

Current RN or LPN state license.
Current CPR card.
Driver’s license.
Social Security card.
Auto insurance card.
TB test results: a two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last five years.
Strong spoken and written communication skills.

Responsibilities

Observe patients, record significant conditions and reactions, and notify a supervisor or physician about the patient’s condition, reactions to drugs or treatments, and significant incidents.
Demonstrate understanding of organizational and nursing policies and procedures.
Communicate effectively, both verbally and in writing.
Manage and direct patient care according to the patient care plan and update the plan as needed.
Administer prescribed medications and treatments, and monitor and document the effectiveness of interventions.
Respond as applicable to changes in a patient’s condition and report changes to the appropriate physician or supervisor.
Assist patients with daily living activities and support their comfort and overall well-being.
