Registered Nurse / Licensed Practical Nurse (RN/LPN)
About The Position
Elite Care Management is hiring a Registered Nurse or Licensed Practical Nurse for part-time home health care in a patient's home in Riverwoods. The role provides one-on-one care to a single patient for the entire shift, helping the patient with daily living activities, comfort, and overall well-being. The nurse will learn about the patient and family’s needs and plan, organize, and assist with home health care services focused on that patient. The listed shifts are 7 a.m. to 7 p.m. on Tuesdays and Thursdays. The employer offers paid orientation and patient training, weekly pay by direct deposit, 24/7 on-call support, and access to a hands-on training lab.
Requirements
- Current RN or LPN state license.
- Current CPR card.
- Driver’s license.
- Social Security card.
- Auto insurance card.
- TB test results: a two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last five years.
- Strong spoken and written communication skills.
- Ability to manage and direct patient care according to a patient care plan.
Responsibilities
- Assist patients with daily living activities and support their comfort and overall well-being.
- Observe patients, record significant conditions and reactions, and notify a supervisor or physician about patient conditions, responses to drugs or treatments, and significant incidents.
- Follow organizational and nursing policies and procedures.
- Communicate effectively in spoken and written form.
- Manage and direct patient care according to the patient care plan, updating the plan as needed.
- Administer prescribed medications and treatments, and monitor and document the effectiveness of interventions.
- Respond as appropriate to changes in a patient’s condition and report changes to the appropriate physician or supervisor.
- Plan, organize, and assist with home health care services for the assigned patient.
