About The Position

Elite Care Management is hiring Registered Nurses or Licensed Practical Nurses for home health care. The role provides one-on-one care to a single patient for the complete shift in the patient's home. Nurses get to know patients and their families, learn about their needs at home, and plan, organize, and assist with home health care services. Nurses assist with daily living activities, ensure patients’ comfort, and support their overall well-being and quality of life. The posting lists full-time and part-time opportunities, with shifts from 7 a.m. to 7 p.m. on various weekdays and weekends. Pay is $35–$40 per hour.

Requirements

• Current RN or LPN state license.
• Current CPR card.
• Driver’s license.
• Social Security card.
• Auto insurance card.
• TB test results: two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last five years.
• Strong spoken and written communication skills.
• Ability to manage and direct patient care according to a care plan and update it as needed.

Responsibilities

• Observe patients, record significant conditions and reactions, and notify a supervisor or physician about patient conditions, reactions to drugs or treatments, and significant incidents.
• Demonstrate understanding of organizational and nursing policies and procedures.
• Communicate effectively both verbally and in writing.
• Manage and direct patient care according to the patient care plan and update the plan as needed.
• Administer prescribed medications and treatments, and monitor and document the effectiveness of interventions.
• Respond as appropriate to changes in a patient’s condition and report changes to the appropriate physician or supervisor.
• Assist patients with daily living activities and support their comfort, well-being, and quality of life.
• Plan, organize, and assist with home health care services for one patient during the shift.
