About The Position

Elite Care Management is hiring a Registered Nurse or Licensed Practical Nurse to provide one-on-one home health care to a patient in Riverwoods. The nurse works with one patient for the entire shift, becoming familiar with the patient and family and their needs at home. The role involves planning, organizing, and assisting with home health services, supporting daily living activities, comfort, and overall well-being to enhance the patient’s quality of life. The part-time schedule is 7 a.m. to 7 p.m. on Tuesdays and Thursdays. Paid orientation and patient training, a hands-on training lab, and 24/7 on-call support are offered.

Requirements

Current RN or LPN state license.
Current CPR card.
Driver’s license.
Social Security card.
Auto insurance card.
TB test results: a two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last five years.
Strong spoken and written communication skills.
Ability to manage and direct patient care according to a care plan.

Responsibilities

Observe the patient, record significant conditions and reactions, and notify a supervisor or physician of the patient’s condition, reactions to drugs or treatments, and significant incidents.
Follow organizational and nursing policies and procedures.
Communicate effectively, both verbally and in writing.
Manage and direct patient care according to the patient care plan and update the plan as needed.
Administer prescribed medications and treatments, and monitor and document the effectiveness of interventions.
Respond as applicable to changes in the patient’s condition and report changes to the appropriate physician or supervisor.
Assist the patient with daily living activities and support their comfort and overall well-being.
