About The Position

Provide skilled nursing care to patients in their homes through intermittent visits, private duty shifts, and care plan coordination. The role is described as flexible and per diem, and is intended for nurses licensed in Florida who can provide reliable, professional care.

Requirements

• Active Florida RN or LPN license; Compact License accepted.
• Current CPR certification (BLS).
• Level 2 background screening that is AHCA-compliant.
• TB screening and physical completed within the past 6 months.
• Familiarity with electronic medical records, such as Axxess.
• For RNs, knowledge of PDGM and OASIS.
• Excellent clinical judgment, communication skills, and time management.
• Reliable transportation and commitment to punctual, professional care.

Responsibilities

• Perform skilled nursing visits, including wound care, IV administration, medication management, and patient assessments.
• Provide shift-based care for patients who need extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines; RN OASIS documentation is required.
• Follow infection control standards and agency protocols.
