This role involves providing comprehensive hospice care to terminally ill patients, focusing on enhancing their quality of life. The Registered Nurse (RN) will function as a primary nurse and a core member of the Interdisciplinary Group, developing and implementing care plans that address the physical, psychosocial, emotional, and spiritual needs of patients and their families. Responsibilities include pain and symptom management, educating patients and caregivers about end-of-life care, and collaborating with physicians and healthcare teams to provide supportive psychotherapy and mental health support. The RN will also assess eligibility for hospice care, obtain informed consent, and ensure compliance with regulatory conditions of participation. This position requires maintaining professional knowledge, adhering to infection control policies, managing patient records, and supervising personal care provided by Home Health Aides.
