About The Position

This is a travel Registered Nurse position specializing in Long-Term Care (LTC) in Middlebury, Vermont. The assignment is for a 98-bed Post-Acute Care unit within a Nursing Home setting. The role involves providing direct nursing care to adults in a post-acute skilled nursing rehab environment, administering medications, monitoring patient status, supporting IV therapy, and collaborating with the care team. The position requires flexibility to float to related units and work night shifts.

Requirements

Active Registered Nurse license in good standing (state-specific)
Minimum 1 year of clinical experience
1+ year in post-acute/skilled nursing rehab preferred
EPIC electronic medical record experience within the past 2 years required
BLS (Basic Life Support) certification
Ability to work 3x12 night shifts, float to other units as needed, and work all holidays
Capacity to work in a post-acute care/unit setting with 60+ age population and to collaborate with a multidisciplinary team

Responsibilities

Provide direct nursing care to adults in a post-acute skilled nursing rehab setting, following physician orders and facility protocols
Administer medications and document in the electronic medical record system (EPIC); perform phlebotomy and EKGs as needed
Monitor patient status, assess changes, respond to alarms, and communicate with the care team regarding patient needs
Support IV therapy, including starting IVs and managing IV pumps, as appropriate
Coordinate patient transports, assist with nebulizer therapy, and support memory care or dementia-capable patients as indicated
Float to related units as needed and collaborate with LNA/CNA/PCT staff; participate in roundings and handoffs
Maintain accurate documentation, ensure patient safety, and comply with infection control and safety standards
