About The Position

This travel assignment is for a Registered Nurse specializing in Long-Term Acute Care (LTAC) on a hospital unit in Rochester, New York. The contract duration is 17 weeks, with 12-hour shifts. The role focuses on geriatric long-term care with wound care needs. The nurse will work with approximately 20 residents, supervising LPNs and CNAs, and providing direct patient care in a hospital-based LTAC setting. No tracheostomy care or acute care requirements are identified for this specific unit.

Requirements

Active NY Registered Nurse license (NY RN license only)
Graduate of an accredited RN program; eligible to practice in New York
Two (2) years of clinical experience in long-term care
BLS certification (American Heart Association)
Ability to work in a geriatric LTAC setting
No tracheostomy care required on the described unit

Responsibilities

Provide direct nursing care for geriatric residents on the LTAC unit, following the established plan of care and physician orders.
Perform comprehensive assessments, monitor vitals, document findings, and update the nursing care plan as needed.
Administer prescribed medications and coordinate medication administration with LPNs and CNAs.
Oversee and support LPN and CNA colleagues, ensuring safe task delegation and patient safety.
Conduct wound care as appropriate, monitor for complications, and document progress.
Maintain accurate documentation, communicate with the care team, and support care coordination across shifts.
