About The Position

This travel RN role provides skilled nursing care in patients’ homes, coordinating with the interdisciplinary team to support quality, safe care. The assignment is in South Portland, Maine, with a start date of 09/21/2026 and a duration of 14 weeks. The role offers a weekly pay range of $2,471–$2,528 with guaranteed 40 hours per week and an 8-hour shift.

Requirements

Active RN license in the applicable state – Required
Graduate of an accredited ADN or Nursing Diploma program; BSN preferred – Required/Preferred
1-2 years RN clinical nursing experience; 2+ years home health experience – Required
Proof of current CPR certification – Required
CHPN certification – Preferred
Valid driver’s license with reliable transportation and auto insurance – Required

Responsibilities

Conduct initial and ongoing assessments of home health patients to identify physical, psychosocial, and environmental needs; develop and adjust the Plan of Care accordingly.
Implement, document, and evaluate the Plan of Care; maintain up-to-date patient records and ensure clear communication of changes.
Deliver skilled nursing procedures (e.g., wound care, IV therapy, phlebotomy, pain management) and manage indwelling devices; administer medications per physician orders.
Educate patients and families on care plans, medications, safety, home safety, and self-care strategies; tailor teaching to age, condition, and cultural context.
Coordinate with the interdisciplinary team; supervise and provide clinical direction to HHA/STNA/LVN; arrange referrals; participate in team meetings; provide on-call support as assigned; ensure payer documentation and requirements are met.
Maintain infection control practices per OSHA standards and safeguard patient confidentiality; communicate effectively with patients, families, physicians, and other health professionals.
