About The Position

This travel position provides skilled nursing care in patients’ homes, coordinating with the interdisciplinary team and families to manage and adjust the Plan of Care as needed. The role involves assessing patients, developing and implementing care plans, maintaining patient records, providing education, supervising support staff, managing pain, arranging referrals, participating in team meetings, and performing on-call responsibilities.

Requirements

  • Current state license as a Registered Nurse – Required
  • CPR certification – Required
  • OASIS experience – Required
  • Homecare Homebase software experience – Required
  • Current driver’s license – Required; reliable transportation and insurance
  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
  • 1–2 years as an RN in a clinical care setting – Required
  • 2+ years of home health experience – Required

Nice To Haves

  • BSN – Preferred
  • CHPN certification – Preferred

Responsibilities

  • Assess home health and hospice patients, in stable and emergent situations, and document observations to identify physical, psychosocial, and environmental needs.
  • Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
  • Obtain and integrate data on physical, psychological, social, spiritual, and cultural factors into the POC.
  • Maintain up-to-date patient records; ensure visits, contacts, problems, plans, actions, and goals are accurately recorded and updated.
  • Provide care with appropriate infection control measures; educate patients and families on care, medications, safety, and self-care practices.
  • Supervise and provide clinical direction to Home Health Aide (HHA), STNA, and LVN staff as needed to maintain care continuity.
  • Assess and reassess pain; implement and educate on pain management strategies.
  • Arrange referrals to appropriate agencies when indicated; monitor third-party payer requirements.
  • Participate in interdisciplinary team meetings and communicate effectively with patients, families, and healthcare professionals.
  • Perform on-call responsibilities as assigned and respond to urgent needs in the home setting.
