About The Position

This is a travel assignment for a Registered Nurse specializing in Home Health (Hospice) care in Newport, Vermont, through Healthforce Healthcare LLC. The assignment is for approximately 25 weeks, starting on 09/21/2026. The role involves providing skilled nursing care to patients in their homes, including assessments, medication administration, and patient/family education. The nurse will be responsible for OASIS data collection and documentation using the HealthWyse (Careficient) EMR, coordinating with an interdisciplinary team, and traveling between patient homes. Some weekend coverage and on-call duties may be scheduled on an emergency basis.

Requirements

Active Registered Nurse license in Vermont
OASIS experience
Home Health and Hospice experience
Experience traveling between patient homes for home visits; ability to manage schedule changes
Valid driver’s license in good standing and reliable transportation
BLS certification and proficiency with HealthWyse (Careficient) EMR (or willingness to learn)

Responsibilities

Provide skilled nursing care to patients in their homes, including assessments and ongoing monitoring
Perform OASIS data collection and documentation within the HealthWyse (Careficient) EMR
Administer medications, manage therapies, and implement individualized care plans
Coordinate with an interdisciplinary team to adjust care as patient needs change
Educate patients and family caregivers on disease processes, medications, safety, and self-care
Travel between patient homes for scheduled visits and respond to urgent needs as assigned while maintaining accurate documentation
