About The Position

This is a travel assignment for a Registered Nurse specializing in Geriatric care in Glendale, CA. The contract is for 13 weeks, with a guaranteed 36 hours per week. The role involves leading patient care coordination, ensuring alignment of care plans with patient needs, and collaborating with physicians and case management. The nurse will focus on complex patients, high ED utilizers, and chronic care needs, coordinating transfers and supporting data collection. The position is in an acute care hospital setting with a day shift schedule from Sunday to Thursday.

Requirements

Active Registered Nurse (RN) license in California
Basic Life Support (BLS) certification from an approved vendor
RN Case Management (CM) certification required
Minimum 2 years of recent RN experience in the field
Inpatient case management experience preferred

Responsibilities

Lead coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.
Ensure the interdisciplinary care plan aligns with the patient’s clinical course, continuing care needs, and covered services; monitor diagnostics, treatments, and procedures.
Discuss with physicians the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge planning.
Focus on complex patients, high ED utilizers, and chronic care needs; collaborate with case management for ongoing care adequacy.
Coordinate transfers to tertiary centers as needed and ensure transfer of patient information for continuity of care; support regulatory and accreditation data collection.
